Pharmacodynamics

Actively acting component of the drug is a substance of non-steroidal nature, which has a pronounced antipyretic, anti-inflammatory and analgesic effect. The main mechanism is the inhibition of the biological synthesis of prostaglandins, which play a leading role in the genesis of pain, inflammation and fever. This preparation does not exert a suppressive effect on the biosynthesis of proteoglycans of the cartilaginous tissue.

In the treatment of pathologies of rheumatic nature, the severity of pain, morning stiffness and swelling of articular joints are significantly reduced, and their functioning is noted to improve.

With inflammatory phenomena of post-operation or post-traumatic origin, the drug also has an analgesic effect. At the same time, he is able to eliminate pain and reduce the severity of blood loss in primary dysmenorrhea. Cures migraine attacks.

Pharmacokinetics

Tablets, covered with an enteric-coated shell, after ingestion are completely absorbed in the intestine. In the event that the medication is taken during or immediately after a meal, its slow passage through the stomach is noted, but the amount of the active ingredient is not changed.

After a single dose of 50 mg of Voltaren, the maximum concentration of diclofenac in the plasma (1.5 μg / ml) is observed after 2 hours. The degree of absorption of the drug depends on the size of the dose. With strict adherence to the intervals between tablets, cumulation is not observed.

The connection with plasma proteins is 99.7% (99.4% - with albumin). In synovial fluid, diclofenac reaches its maximum concentration 2-4 hours later than in plasma. The half-life of the drug in synovia is 3-6 hours.

Metabolism of diclofenac is carried out in the liver, with the formation of phenolic metabolites, which turn into glucuronide conjugates.

The total plasma clearance of the active ingredient is 263 plus or minus 56 ml / min. The final half-life is 1-2 hours. T 1/2 metabolites - 1-3 hours. About 60% of the drug is excreted by the kidneys along with the urine, the rest - along with the feces.
