Avelox is an antibacterial drug that belongs to the group of fluoroquinolones and has a bactericidal effect on microorganisms sensitive to it.

This drug has a wide range of antibacterial activity against not only Gram-positive and Gram-negative bacteria, but also anaerobes, atypical pathogens and acid-resistant microorganisms.

Active ingredient: Moxifloxacin hydrochloride.

Release form

  • In the form of a yellow solution for intravenous infusion. 1 vial of the drug contains 436 mg of moxifloxacin hydrochloride, which is equivalent to 400 mg of moxifloxacin.
  • In the form of tablets containing 400 mg of active ingredient.

Mechanism of action

Avelox contributes to the death of microorganisms by suppressing the activity of topoisomerases, which are involved in the formation of DNA molecules.

Spectrum of antimicrobial activity: Avelox is effective in cases where the pathogen is resistant to beta-lactam class antibiotics and macrolides.

The spectrum of the antimicrobial activity of the drug is as follows:

  • Gram-positive pathogens: Streptococcus pneumoniae, Gardnerella vaginalis, Streptococcus pyogenes, Staphylococcus aureus, Staphylococcus epidermidis, Staphylococcus haemoliticus, Corynebacterium diphtheria.
  • Gram-negative pathogens: Haemophylus influenza, Moraxella catarrhalis, Bordetella pertussis, Klebsiella pneumoniae, E.coli, Proteus vulgaris.
  • Anerobic bacteria: Bacteroides spp., Fusobacterium spp., Peptostreptococcus spp., Clostridium perfringens.
  • Atypical pathogens: Chlamidia pneumoniae, Chlamidia trahomatis, Mycoplasma pneumoniae, Legionella pneumophilia.

The development of cross-resistance mechanisms between Avelox and antibiotic groups such as aminoglycosides, macrolides, tetracyclines and penicillins is not observed.

Suppression of resistance mechanisms in a bacterial cell is achieved by the fact that a molecule is laid in the structure of Avelox, which prevents the so-called efflux (i.e., the release of the fluoroquinolone group drug from the microorganism cell).

Pharmacokinetics

Absalox bioavailability is 91%. In the systemic circulation, Moxifloxacin is 45% bound to plasma albumin. The highest concentrations of the drug, regardless of the route of administration, are noted in the tissue of the lungs, paranasal sinuses, saliva, extracellular fluid, as well as foci of inflammation.

The main route of elimination of Avelox is renal, while the excreted metabolites are considered inactive in relation to the human body.

Indications for use

The drug is used for the treatment of bacterial diseases caused by microorganisms sensitive to Avelox:  

  • Community-acquired pneumonia (including those caused by multi-resistant strains);
  • Infected skin and soft tissue lesions (burn, wound, diabetic);
  • Intra-abdominal infectious inflammatory processes (abscesses, etc.).

Dosing and Administration

The drug is administered intravenously at a dose of 400 mg once a day.

It is rational to use a staggered pharmacotherapy regimen, which implies that after 3-4 days of intravenous infusions are being transferred to a pill intake of an antibacterial agent.

Patients older age group dose adjustment is not performed.

The average duration of treatment

  • Chronic bronchitis in the period of exacerbation - 5 days;
  • Community-acquired pneumonia (parenteral + oral antibiotic) - 7-14 days;
  • Complicated soft tissue infections - 7-21 days;
  • Infections of the abdominal cavity 5-14 days.

Terms of use of the drug

Avelox is administered intravenously for at least one hour. The following solutions for infusion are compatible with Moxifloxacin: sodium chloride, glucose, Ringer's solution, xylitol.

In the case when combined intravenous therapy is carried out, the remaining drugs are injected in a separate syringe.

If during the storage of Avelox in the refrigerator a precipitate formed in it, then the solution is suitable for use. However, it should be warmed at room temperature until the precipitate is completely dissolved.

Tablets of the drug are used, regardless of the meal 1 time per day. It should be noted that it is impossible to break the integrity of the outer shell of the tablet (crushed or crushed).


Side effect

  • Cardiovascular system: reduction of blood pressure, the occurrence of arrhythmias, cardiac arrest (most often occurs in individuals with ischemic myocardial lesions).
  • The digestive system: a complex of dyspeptic disorders, increased levels of transaminase liver fractions, stomatitis, pseudomembranous colitis, hepatitis.

Contraindications

  • Breast-feeding.
  • Pregnancy at any time.
  • Intolerance to the active substance or other components of the drug.
  • Age less than 18 years.

Overdose

It was established experimentally that the administration of the drug in doses of 1200 mg and larger once for 10 days was not accompanied by clinical signs of intoxication.

Interaction with other drugs

Caution should be exercised in the combined use of Avelox with anticoagulants because of the possibility of potentiation of the action of the latter.

It is impossible to combine Moxifloxacin and sodium bicarbonate solution in one syringe.

Milk and other food components do not alter the pharmacokinetics of the drug.

Storage conditions

Avelox should be stored at a temperature greater than 15 0 C.
