Asentra: instructions for use
Asentra (Asentra) refers to the modern drugs of the group of antidepressants and helps to cope with panic conditions.
Form of issue and composition
the drug is released in the form of white tablets, the edges of which are chamfered. On one side of the tablet there is a notch.
One tablet contains:
55.95 mg sertraline hydrochloride, equivalent to 50 mg sertraline;
111.9 mg of sertraline hydrochloride, equivalent to 100 mg of sertraline.
Auxiliary components: talc, calcium dihydrogen phosphate, magnesium stearate, microcrystalline cellulose, sodium starch glycolate, hydroxypropylcellulose.
Contoured cell packs of 7 tablets are in a cardboard box with 4 blisters.
Mechanism of action
A low serum neuronimiator level is one of the biochemical causes of depression, panic and anxiety. Asentra (sertraline) inhibits the re-uptake of serotonin from platelets, maintaining its optimal concentration in the body. At the same time sertraline does not have a stimulating, sedative effect and does not block the transmission of nerve impulses.
The antidepressant effect of sertraline is observed after two weeks of regular use, and the maximum effect is manifested after 1.5 months of its use. Unlike representatives of tricyclic antidepressants (desipramine, amitriptyline), the appointment of sertraline does not cause an increase in body weight. The positive side of sertraline is that the drug is devoid of mental or physical dependence.
After taking sertraline, the maximum concentration in the blood is observed after 4-9 hours, because the drug is slowly absorbed from the gastrointestinal tract. A balanced concentration of sertraline in the body is observed one week after the start of the drug. If the drug is taken with food, then the rate of its absorption increases by 25%.
Indications for use
- treatment and prevention of various forms of depressive conditions, including those accompanied by anxiety;
- obsessive-compulsive disorder;
- panic conditions;
- stressful posttraumatic disorders.
- allergy and individual intolerance of the active substance or auxiliary components;
- use of monoamine oxidase inhibitors (MAO);
- joint use of sertraline with preparations of tryptophan or fenfluramine;
- epilepsy of unstable nature;
- pregnancy and lactation;
- children's age till 6 years.
Dosing and Administration
Sertraline is used orally.
During the period of depression and obsessive-compulsive disorders, the drug is taken in the morning or evening once in a dose of 50 mg. If necessary, the dose may be increased to 200 mg per day, but not earlier than 7 days after the beginning of the application.
For the treatment of panic and post-traumatic disorders, Asentra is prescribed at an initial dose of 25 mg once a day in the morning or evening. The dose of sertraline can be increased one week after application to 50 mg. Further increase in the dose to 200 mg per day can be done gradually, after a week.
To ensure a satisfactory therapeutic effect of sertraline, usually one week of use is sufficient. A stable therapeutic effect is provided after 3-4 weeks of use. Patients with obsessive-compulsive disorders in certain situations require a longer course of therapy - up to 12 weeks.
In pediatrics, the initial dose of sertraline for children from 6 to 12 years does not exceed 25 mg. The drug is taken in the morning or evening once. The increase in the dose of the substance can be in a week to 50 mg per day. At the age of 12 to 17 years, the use of sertraline begins with 50 mg once a day, and after a week the dose may increase to 200 mg. However, the doctor must take into account the individual weight of the child and avoid the likelihood of an overdose of the drug.
Elderly patients do not require an individual adjustment of sertraline dose.
From the gastrointestinal tract : decreased or no appetite, in rare cases, anorexia; a feeling of dryness in the mouth; dyspepsia (increased gassing in the intestine, nausea, vomiting, constipation, or diarrhea); spastic pain in the abdominal cavity, hepatitis, hepatic insufficiency or jaundice, pancreatitis .
From the nervous system : lethargy and drowsiness; insomnia; dizziness; bursts of emotional arousal with a sense of anxiety and fear; tremor; manifestation of manic behavior in various degrees; a restlessness syndrome with an internal need to constantly move; sensitivity disorder, numbness of individual parts of the body; convulsions ; violation of gait; aggression; hallucinations; excitation; psychosis; symptoms of depression.
From the genitourinary system : erectile dysfunction; decreased libido; delay in ejaculation; anorgasmia, benign enlargement of mammary glands in men; menstrual cycle disorders in women; prolonged painful erection; increase in the level of the hormone prolactin in the blood; pathological spontaneous discharge of milk from the chest.
On the part of the respiratory organs : a feeling of lack of air and squeezing of the chest, suffocation.
From the cardiovascular system : an increase in the number of heartbeats; increased blood pressure; pain in the chest; the appearance of puffiness; rarely: fainting and tachycardia .
From the sense organs : fuzzy visual perception.
Allergic manifestations : urticaria ; edema in the eyelids, lips and face; rash on the skin; redness of the skin; systemic itching; exudative erythema multiforme.
In addition, there may be significant changes in the data of laboratory analyzes of the enzyme system of the body, blood, hormonal background.
With an overdose of sertraline, there is an increase in its side effect due to an increase in the toxic effect of the drug.
Sertraline requires extreme caution in the following situations:
- mental retardation and neurological disorders;
- loss of body weight;
- manic states;
- children age over 6 years.
In case of hepatic or renal insufficiency, the doctor should take into account the fact of delay of sertraline in the body and individually adjust the dose of the drug.
The interval should be observed when using MAO inhibitors and sertraline . If there is a need to replace one drug with another, then from the use of antidepressants abstain for at least two weeks.
If the patient takes other medications other than sertraline, then this should be reported to the doctor. There is a possibility of distortion of the therapeutic effect or the appearance of undesirable side effects.
The use of sertraline during pregnancy is advisable if the benefits of using it significantly exceed the harm for the unborn child. Women of childbearing age during the use of sertraline should resort to effective methods of contraception. If sertraline should be used during lactation, then breastfeeding should be discarded, since the drug is excreted with milk.
If there is a tendency to suicidal attempts , then until the stable therapeutic concentration of the substance in the body is reached (about 14 days), the patient should be constantly monitored.
Sertraline monotherapy does not affect the ability to drive vehicles and activities where increased concentration is required. In cases where several drugs are used simultaneously, there is a risk of disturbing psychomotor reactions and vehicle management should be discarded.
