Assentra: instructions for use
Assentra (Asentra) belongs to the modern drugs of the antidepressant group and helps to cope with panic conditions.
Release form and composition
the drug is produced in the form of white tablets, the edges of which are beveled. On the one hand the pill has a notch.
One tablet contains:
55.95 mg sertraline hydrochloride, which is equivalent to 50 mg sertraline;
111.9 mg sertraline hydrochloride, equivalent to 100 mg sertraline.
Auxiliary components: talc, calcium dihydrophosphate, magnesium stearate, microcrystalline cellulose, sodium starch glycolate, hydroxypropylcellulose.
The blister packs of 7 tablets are in a carton of 4 blisters.
Mechanism of action
Low levels of the neuromidiator serotonin is one of the biochemical causes of depression, panic, and anxiety. Ascentra (sertraline) inhibits the reuptake of serotonin from platelets, maintaining its optimal concentration in the body. At the same time, sertraline does not have a stimulating, sedative effect and does not block the transmission of nerve impulses.
The antidepressant effect of sertraline is observed after two weeks of regular use, and the maximum effect appears after 1.5 months of its use. Unlike representatives of tricyclic antidepressants (desipramine, amitriptyline), the administration of sertraline does not cause an increase in body weight. The positive side of sertraline is that the drug is devoid of mental or physical dependence.
After taking sertraline inside the maximum concentration in the blood is observed after 4-9 hours, because the drug is slowly absorbed from the gastrointestinal tract. A balanced concentration of sertraline in the body is observed a week after the start of the use of the drug. If the drug is taken with food, the rate of its absorption increases by 25%.
Indications for use
- treatment and prevention of various forms of depression, including those that are accompanied by anxiety;
- obsessive-compulsive disorder;
- panic states;
- stressful post-traumatic disorders.
- allergy and idiosyncrasy of the active substance or auxiliary components;
- the use of monoamine oxidase inhibitors (MAO);
- co-administration of sertraline with tryptophan or fenfluramine preparations;
- epilepsy of unstable nature;
- pregnancy and lactation;
- children's age up to 6 years.
Dosing and Administration

In the period of depression and obsessive-compulsive disorders, the drug is taken in the morning or evening once in a dose of 50 mg. If necessary, the dose can be increased to 200 mg per day, but not earlier than 7 days after the start of use.
For the treatment of panic and post-traumatic disorders, Ascenro is prescribed in an initial dose of 25 mg once a day, in the morning or evening. The dose of sertraline can be increased a week after application to 50 mg. A further increase in the dose to 200 mg per day can be made gradually, in a week.
To ensure a satisfactory therapeutic effect of sertraline, usually one week is enough. Stable therapeutic effect is provided after 3-4 weeks of use. Patients with obsessive-compulsive disorders in certain situations require a longer course of therapy - up to 12 weeks.
In pediatrics, the initial dose of sertraline for children from 6 to 12 years does not exceed 25 mg. The drug is taken in the morning or in the evening once. Increasing the dose of a substance can be up to 50 mg per day in a week. At the age of 12 to 17 years, the use of sertraline begins with 50 mg once a day, and after a week the dose may increase to 200 mg. However, the doctor must take into account the individual weight of the child and avoid the likelihood of an overdose of the drug.
Elderly patients do not require individual dose adjustment of sertraline.
On the part of the gastrointestinal tract : a decrease or lack of appetite, in rare cases anorexia; feeling of dry mouth; dyspepsia (increased flatulence in the intestines, nausea, vomiting, constipation, or diarrhea); abdominal cramps, hepatitis, liver failure or jaundice, pancreatitis .
On the part of the nervous system : lethargy and drowsiness; insomnia; dizziness; bursts of emotional arousal with feelings of anxiety and fear; tremor; the manifestation to varying degrees of manic behavior; restlessness syndrome with an internal need to constantly move; sensitivity disorder, numbness of certain parts of the body; convulsions ; gait disturbance; aggression; hallucinations; excitation; psychosis; symptoms of depression.
From the genitourinary system : erectile dysfunction; decreased libido; delay ejaculation; anorgasmia, benign breast enlargement in men; menstrual disorders in women; prolonged painful erection; increased levels of the hormone prolactin in the blood; pathological spontaneous discharge of milk from the breast.
On the part of the respiratory system : a feeling of lack of air and squeezing of the chest, suffocation.
Since the cardiovascular system : an increase in the number of heartbeats; high blood pressure; chest pain; the appearance of puffiness; rarely: fainting and tachycardia .
On the part of the senses : blurred visual perception.
Allergic manifestations : urticaria ; swelling in the eyelids, lips and face; skin rashes; redness of the skin; systemic itching; exudative erythema multiforme.
In addition, there may be significant changes in the data of laboratory tests of the body's enzyme system, blood, hormonal levels.
With an overdose of sertraline, an increase in its side effects is observed due to an increase in the toxic effect of the drug.
Sertralin requires special care in the following situations:
- mental retardation and neurological disorders;
- weight loss;
- manic states;
- child age over 6 years.
In case of hepatic or renal failure, the doctor should take into account the fact of sertraline delay in the body and individually adjust the dose of the drug.
The interval should be observed when using MAO inhibitors and sertraline . If there is a need to replace one drug with another, then refrain from using antidepressants for at least two weeks.
If the patient is taking other drugs besides sertraline, then this should be reported to the doctor. There is a possibility of a distortion of the therapeutic effect or the appearance of unwanted side effects.
The use of sertraline during pregnancy is advisable if the benefits of its use far exceed the harm for the unborn child. Women of childbearing age when using sertraline should resort to effective methods of contraception. If sertraline needs to be used during lactation, then breastfeeding should be discarded, since the drug is excreted in milk.
If there is a tendency to suicidal attempts , then until a stable therapeutic concentration of the substance in the body is reached (about 14 days), the patient should be under constant medical supervision.
Monotherapy with sertraline does not affect the ability to drive vehicles and activities that require increased concentration of attention. In cases where several drugs are used at the same time, there is a risk of impaired psychomotor reactions and driving control should be abandoned.
